We have a tradition at our annual holiday party to celebrate everyone's acheivements for the year by handing out certificates of achievement, suitable for display, listing your dog's achievements for the year, but only if you specify what you'd like to be recognized for (the Club has no good way of tracking this).

Use this form to e-mail us your information. Fill out the form for each dog by (DEADLINE!) Wednesday Jan-7, 6PM.

By the way if you are wondering what happened to the club's tradition of also including this information in an annual Clean Run magazine bragging ad, this year we're opting to instead produce a club calendar, complete with photos of club dogs and agility dates annotated. If you wish to submit a photo, get details from info@bayteam.org.

Dog's Name:

Dog's name exactly as it should appear on the certificate (either callname or registered name, whatever you want), WITH ALL titles exactly as they and the name should appear on the certificate. AKC, CPE, DOCNA, TDAA, UKC, NADAC, USDAA, all organizations accepted, all sports accepted.
